Department of Defense Health Care Integration

In our work with the Health Care Integration award through the Department of Defense, StrategyGen Co. (SBA Certified HUBZone Small Business), with great work from Jon Thomas, has many responsibilities. here is the latest summary of work at MacDill AF Base. Chart Review and Colon Cancer Screening Letters: Reviewed 1328 charts of due/overdue patients between the ages of 50-74. Sent 1188 letters to patients. The activity aims to prevent or detect colon cancer early.

TBI Project: Managed/submitted 10 cases for approval into a joint venture program with the Tampa VA. The project focuses on case management for ADSMs with past exposure to TBI instances to determine eligibility for the program.

Telephone Consults: Received 44 phone calls from patients with requests for screening tests. Created 7 telephone consults. The activity aims to prevent or detect illness/disease early. Chart Review and A1c Screening Letters: Reviewed 64 charts of due/overdue patients between the ages of 50-74. Sent 64 letters to patients. The activity aims to prevent or detect diabetes early.

Facilitated Population Health Meeting: The Population Health Working Group is a multidisciplinary team that exists to develop strategies to address the health and healthcare needs of the population within its area of influence, e.g. military medical treatment facility empanelment. The monthly meeting with the Chief of Medical Staff, Health Care Integrator, Case Manager, Disease Manager, Utilization Manager, and Special Needs Coordinator aims to discuss progress toward medical management goals (e.g., from the population health plan, ongoing process improvement initiatives), complex patient cases, status of referrals to appropriate level of care, and lessons learned.

Pediatric PCM change letters: Created change of PCM letter for 482 peds patients, prepped for mailing. This activity is the final step of the empanelment project for new SGH realignment to new PCMs.

Diabetic Rodeo: The activity is in the beginning stages of a quarterly clinic for diabetic well-checks. Senior staff at ECOMS meetings were briefed to notify of intention and gain support of stakeholder involvement. Several providers expressed interest in being involved, and there was a green light from SGH and GPM to move forward.
